Cole hears the hurt and is driven to try (sometimes quite badly) to untangle it.
This means he primarilly hears or sees or otherwise reads memories and thoughts that cause the person pain; those are the strongest. Canon banter has him picking up on other things as well -- one need look no further than his relationship banters either about the Inquisitor/love interest, or for example Dorian/Iron Bull. But pain is the loudest, the brightest, the sharpest. Regrets, traumas, anxieties.
( Fun side note: sometimes he can also pick up on the thoughts and feelings of someone who isn't even there; whole countries away, if they're involved strongly in the memory of the person he's reading. He did this to Bull at one point in banter. Things like physical distance aren't very relevant to a spirit. )
Give me something Cole can pick up on; a description (I'll tweak it into his sometimes peculiar wording), a quote for him to repeat aloud + some context to work with since he'd know the situation it was said in. Tell me if the pain would feel very distant or very immediate or where it might lie between. Your character does not have to be thinking about it at the time for Cole to pick up on it!
It can be something your character doesn't even know has been bothering them. It can be a secret, or common knowledge; it doesn't seem to matter to Cole's abilities.
If you want a more general flavor of CR with him that isn't entirely about the mindreading, but you dig the idea of it in general, that's fine too! I can (and should) sprinkle it in without us having to make it the focus. Cole can vary from lancing an emotional wound to just that very peculiar boy, and everything in between.
If I tagged you first, I'm probably assuming Cole won't pick up on anything for whatever reason. And if you'd rather not have him read anything, just don't send me anything and I'll work accordingly. It's opt-in, OOCly!
ICly he could be distracted by others with more pressing pains, and just not hear your character's. Works out for everyone, that way. :)
